Permission to Administer Medication (short form) Fill out this form and print it off to bring with you when your child needs to be given medication at the school. This is required for everything from diaper cream to antibotics.

A background check is required for all employees of the Day School and all those who volunteer with the Day School and for those who volunteer to work with children and youth at the church.
